Overview Of Tadalafil / Apomorphine HCl Troche
Tadalafil Apomorphine HCl Troche is a compounded preparation, made to the strength and directions your prescriber specifies. Prepared in: 40/6 mg.
Prescribers order preparations of this kind for erectile dysfunction and sexual health, usually where a commercial tablet is unsuitable — a different strength, a different route, or a combination prepared for one patient.
All of our compounding is done at Brandon Pharmacy and Compounding and transferred to our other four Florida stores, so you can collect it wherever you normally go. A prescription is required. Compounded preparations are not FDA-approved.

Generic Details
Tadalafil is a selective inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5). It has the empirical formula C22H19N3O4 and a molecular weight of 389.41. It is a crystalline solid, practically insoluble in water and very slightly soluble in ethanol.
Commercially, tadalafil is approved by the FDA as CIALIS for erectile dysfunction, for the signs and symptoms of benign prostatic hyperplasia, and for the two together. The preparation on this page is compounded to a prescriber’s order and is not an FDA-approved product.
Apomorphine hydrochloride
Apomorphine hydrochloride is a non-ergoline dopamine agonist, molecular formula C17H17NO2·HCl·½H2O, molecular weight 312.79. It appears as white or greyish-white glistening crystals, or as a white powder, soluble in water at 80°C.
Its only FDA-approved form is APOKYN, a subcutaneous injection for the acute, intermittent treatment of “off” episodes in advanced Parkinson’s disease. Its use in this troche is a compounded preparation and is not an FDA-approved use.
MOA
Erection during sexual stimulation depends on increased penile blood flow, which follows relaxation of the penile arteries and the smooth muscle of the corpus cavernosum. Nitric oxide released from nerve endings and endothelial cells stimulates production of cGMP in those smooth muscle cells; cGMP causes the muscle to relax and blood to flow in.
Tadalafil inhibits PDE5, the enzyme that breaks cGMP down, so more cGMP remains available. Because sexual stimulation is what triggers the local release of nitric oxide in the first place, inhibiting PDE5 has no effect without it.
PDE5 is also present in the smooth muscle of the prostate and bladder and their blood supply, which is relevant to benign prostatic hyperplasia, though the mechanism by which tadalafil reduces BPH symptoms has not been established. In laboratory studies tadalafil is more than 10,000 times more potent against PDE5 than against PDE1, PDE2, PDE3, PDE4 and PDE7, and roughly 700 times more potent against PDE5 than against PDE6, the enzyme in the retina responsible for phototransduction.
Apomorphine
Apomorphine is a non-ergoline dopamine agonist. It binds with high affinity to the dopamine D4 receptor and with moderate affinity to the D2, D3 and D5 receptors and to the adrenergic α1D, α2B and α2C receptors. In Parkinson’s disease its precise mechanism is unknown, but it is thought to work by stimulating post-synaptic D2-type dopamine receptors in the caudate-putamen.
Clinical Pharmacokinetics
Absorption. After a single oral dose, peak plasma concentration is reached between 30 minutes and 6 hours, with a median of about 2 hours. Food does not affect the rate or extent of absorption. Across the 2.5 to 20 mg range, exposure increases in proportion to dose. With once-daily dosing, steady state is reached within 5 days and exposure is about 1.6 times that of a single dose.
Distribution. The mean apparent volume of distribution is about 63 L, indicating distribution into tissues. At therapeutic concentrations about 94% is bound to plasma proteins. Less than 0.0005% of a dose appears in semen.
Metabolism. Tadalafil is metabolised predominantly by CYP3A4 to a catechol metabolite, which is then extensively methylated and glucuronidated. The major circulating metabolite is the methylcatechol glucuronide, which is not expected to be pharmacologically active at the concentrations observed.
Excretion. Mean oral clearance is 2.5 L/hr and the mean terminal half-life is 17.5 hours in healthy subjects. Elimination is mainly as metabolites, about 61% in the faeces and about 36% in the urine.
Older patients. Healthy men aged 65 and over show about 25% higher exposure with no change in peak concentration. Age alone does not warrant a dose change, though greater sensitivity in some older individuals should be considered.
Pregnancy
Tadalafil is not indicated for use in women, and there are no adequate and well controlled studies of its use in pregnancy.
In animal reproduction studies there was no evidence of teratogenicity, embryotoxicity or fetotoxicity in rats or mice at exposures up to 11 times the maximum recommended human dose of 20 mg/day during organogenesis. In one of two perinatal and postnatal studies in rats, pup survival after birth fell at maternal doses more than 10 times that exposure, and signs of maternal toxicity appeared above 16 times. Surviving offspring developed and reproduced normally. Tadalafil and its metabolites cross the placenta in rats.
Apomorphine
There are no adequate data on developmental risk from apomorphine use in pregnant women. In animal studies it caused increased neonatal deaths in rats and an increased incidence of heart and great-vessel malformations in rabbits at clinically relevant doses, alongside maternal toxicity. The no-effect dose for adverse developmental effects is below the maximum recommended human dose on a body-surface-area basis.
Breast-Feeding
Tadalafil is not indicated for use in women. It is not known whether tadalafil passes into human milk.
Tadalafil or a metabolite of it was secreted into the milk of lactating rats at concentrations roughly 2.4 times those found in plasma, but drug levels in animal milk may not reliably predict levels in human milk.
Drug Interactions
Nitrates — do not combine. Taking tadalafil with any form of organic nitrate is contraindicated, because tadalafil potentiates the blood-pressure-lowering effect of nitrates. If nitrate treatment becomes medically necessary in a life-threatening situation, at least 48 hours should pass after the last tadalafil dose, and the nitrate should still be given only under close medical supervision with haemodynamic monitoring.
Alpha-blockers. PDE5 inhibitors and alpha-adrenergic blocking agents are both vasodilators, so blood pressure effects can add together. Caution is advised; studies have been done with doxazosin, tamsulosin and alfuzosin.
Antihypertensives. Small reductions in blood pressure occurred when tadalafil was given alongside amlodipine, angiotensin II receptor blockers, bendrofluazide, enalapril and metoprolol compared with placebo.
Alcohol. Both alcohol and tadalafil are mild vasodilators. Substantial alcohol intake — 5 units or more — combined with tadalafil can increase the chance of orthostatic symptoms including faster heart rate, a drop in standing blood pressure, dizziness and headache.
Drugs that raise tadalafil levels. Tadalafil is a CYP3A4 substrate. Ketoconazole 400 mg daily increased single-dose exposure by 312%; ketoconazole 200 mg daily increased it by 107%. Ritonavir 200 mg twice daily increased exposure by 124%. Erythromycin, itraconazole and grapefruit juice would be expected to raise exposure as well.
Drugs that lower tadalafil levels. Rifampin 600 mg daily reduced single-dose exposure by 88%. Carbamazepine, phenytoin and phenobarbital would be expected to reduce it similarly.
Antacids and H2 blockers. An antacid slowed absorption without changing overall exposure. Raising gastric pH with nizatidine had no significant effect.
What tadalafil does not affect. Tadalafil did not significantly affect exposure to S-warfarin or R-warfarin, nor warfarin’s effect on prothrombin time; it did not potentiate the increase in bleeding time caused by aspirin; and it had no significant effect on midazolam, lovastatin, digoxin or theophylline pharmacokinetics.

Apomorphine — additional interactions
5HT3 antagonists — contraindicated. Profound hypotension and loss of consciousness have been reported when apomorphine was given with ondansetron. Taking apomorphine with any 5HT3 antagonist — ondansetron, granisetron, dolasetron, palonosetron, or alosetron — is contraindicated. These are common anti-sickness medicines, so tell your prescriber and pharmacist if you take one.
Nitrates. Sublingual nitroglycerin taken with apomorphine produced larger falls in blood pressure than apomorphine alone, with standing systolic pressure dropping by as much as 65 mm Hg in some individuals. Patients taking apomorphine should lie down before and after taking sublingual nitroglycerin.
Antihypertensives and vasodilators. In clinical studies, patients also taking these medicines had more hypotension (10% against 4%), myocardial infarction (3% against 1%), serious falls (9% against 3%) and bone and joint injuries (6% against 2%).
Alcohol. Both high and low doses of alcohol produced larger blood pressure falls with apomorphine than apomorphine alone. Avoid alcohol after taking it.
Dopamine antagonists. Neuroleptics (phenothiazines, butyrophenones, thioxanthenes) and metoclopramide may reduce apomorphine’s effect.
Drugs that prolong the QT interval. Caution is advised when these are taken alongside apomorphine.
Tadalafil / Apomorphine HCl Troche Side Effects & Reactions
Reported in clinical trials. In eight placebo-controlled studies of tadalafil taken as needed, the adverse reactions reported by at least 2% of patients and more often than on placebo were headache, dyspepsia, back pain, myalgia, nasal congestion, flushing and pain in a limb. At 20 mg, headache was reported by 15% and dyspepsia by 10%, against 5% and 1% on placebo. Discontinuation because of an adverse event occurred in 3.1% of patients on tadalafil 10 or 20 mg, against 1.4% on placebo.
Reported after marketing. These reports are voluntary and from a population of unknown size, so neither their frequency nor a causal link to the drug can be established reliably.
- Cardiovascular and cerebrovascular: myocardial infarction, sudden cardiac death, stroke, chest pain, palpitations and tachycardia. Most but not all of these patients had pre-existing cardiovascular risk factors.
- Eyes: visual field defect, retinal vein occlusion, retinal artery occlusion, and rarely non-arteritic anterior ischemic optic neuropathy (NAION), a cause of decreased vision including permanent loss of vision.
- Ears: sudden decrease or loss of hearing.
- Nervous system: migraine, seizure and seizure recurrence, transient global amnesia.

Seek medical help straight away for an erection lasting longer than 4 hours, for sudden loss of vision in one or both eyes, or for a sudden decrease or loss of hearing.
Apomorphine — reported adverse reactions
In placebo-controlled trials in Parkinson’s disease, the most common adverse reactions occurring at least 10% more often than on placebo were yawning, drowsiness and somnolence, dyskinesias, dizziness and postural hypotension, rhinorrhoea, nausea and vomiting, hallucination and confusion, and swelling of the extremities.
The labelling for apomorphine also warns of falling asleep during daily activities, syncope and orthostatic hypotension, falls, hallucinations and psychotic-like behavior, hemolytic anemia, impulse-control problems, and coronary events.
